If you would like to pay by cheque, please download and print the PDF form below. Fill in all the required information and send it by mail with your cheque to the address listed. You may also send in your room measurements, photos and inspiration materials at the same time. You will be notified by email once we have received your payment.

Payment must be made in full before we will begin work on your design.
